The 4Ms Champions Program 
is a peer-led healthy aging program guided
by the 
4Ms Age-Friendly framework 
to promote healthy aging and  dementia-friendly communities at Brooklyn Senior Centers

WHY PEERS?

  • Peers can help promote healthy habits because they share similar life experiences and can relate to each other, making them trustworthy.

 

  • Older adults feel more comfortable with peers who understand their challenges and cultural background, making it easier to talk about health.

 

  • Seeing peers make healthy choices can encourage others to do the same, creating positive change.

 

  • Peer educators explain health information in a way that is easy to understand and useful in daily life.

 

  • Peers use simple words and casual conversations to talk about health, making learning easier.

 

  • Peer support provides encouragement and motivation, helping people stick to good habits like exercising, eating healthy, or quitting smoking.

PROJECT H.E.A.L

will train groups of seniors at participating senior centers to provide healthy aging information and support educational activities.

The 4M Trainings

  • Held every 3-4 months at a senior center.

  • Each center sends 2-3 participants.

  • Focuses on one 4M topic at a time.

What Trainees
Will Learn:

  • How to share information and resources about the 4Ms.

  • How to talk with peers one-on-one or in small groups to build trust.

  • How to provide support and encourage healthy aging.

THE 4Ms CURRICULUM

The 4Ms Champions curriculum includes an overview on the 4Ms, common aging-problems and healthy aging prevention strategies. 

WHAT MATTERS

  • How to communicate with your provider.  

  • Understanding your care plan and labs.  

  • Care Preference

  • Advanced directives

  • Health Care proxy. 

MEDICATION

  • Too many pills.

  • Over-the-counter medications.

  • How to manage your medications.

  • How to reduce your medications.

MENTATION

  • Depression & Anxiety.

  • Delirium Brain Games and Memory Loss.

  • Alzheimer's Disease & Dementia.

  • Dementia-Friendly Communities (DFCs) - Promote inclusivity, safety, and respect for people living with dementia.

MOBILITY

  • Fall risks.

  • Home safety checklist.

  • Problems with how you walk (Gait).

  • Changing physical function / Disability.

  • Feet issues Tools to help you get around (ambulatory aids).

HEALTHY AGING PREVENTION

  • Annual screens.

  • Vaccinations.

  • Diet.

  • Exercise.

  • Social engagement

AGING CHALLENGES (GERIATRIC SYNDROMES) 

  • Bladder Issues.

  • Problems with Sleep.

  • Loss of Muscle (sarcopenia and frailty).

  • Weight Loss (malnutrition).

  • Visual and Hearing loss.
